Healthcare certification for CBD
Your first step in applying to the Iowa Department of Public Health (IDPH) for a CBD card in Iowa to a visit with a healthcare practitioner who can complete the certifying paperwork with you. Schedule an appointment with us today!
What do I need to bring to my appointment?
1) Your driver's license. 2) IDPH certification form. 3) Copy of your medical record stating you are being treated for one of the medical conditions approved by IDPH for treatment with CBD.
How do I get a Medical Cannabidiol registration card in Iowa?
1) Get certified by our Health Care Practitioner. 2) Apply online with the Iowa Department of Public Health. 3) Visit one of the State approved dispensaries.
